Before new visits

NOTE: This consent is only valid for one attempt and must therefore be completed before each return.
Before sperm freezing
Before insemination with donated sperm (couples/singles)
Consent to the introduction of a fertilized egg into the body of the spouse/partner
After birth
Go to 1177 – Livio Gärdet and select Pregnancy follow-up . Here you can use your bank ID to provide us with information regarding the birth. Of course, you can also send in the form that you received in connection with a visit to us, where pregnancy was confirmed. If you would like a new form sent to you, please contact us and we will arrange it!
Destruction of embryos/eggs/sperm
If you wish to destroy embryos/gametes/sperm, it can be done via 1177 – Livio Stockholm
