What is IUI with Donor Sperm?
Insemination (IUI) with donor sperm can be performed in a natural menstrual cycle or combined with treatment that stimulates ovulation. Insemination with donor sperm is usually only done if the person who will carry the pregnancy is younger than 36-37 years old. The older the woman is, the less frequently eggs of good quality are produced, which means it can take a long time before a pregnancy occurs. Generally, IVF is a more effecient treatment than insemination, and the difference becomes greater the older the woman is.
Who is it for?
IUI with donor sperm may be considered when the couple or partner, for various reasons, lack own sperm production or in cases where treatments with own sperm have not produced embryos of sufficient quality and no other treatment is possible. It can also be considered when one partner carries genes for a severe hereditary disease.

Evaluation
The prerequisites are the same as for all other types of assisted reproduction. A woman who is single or in a relationship can undergo treatment. A couple should be married or cohabiting and living in a stable relationship. A fertility evaluation of the person wishing to become pregnant should be done before treatment if not done previously. The evaluation includes a doctor's visit with medical history, gynecological examination with ultrasound, and testing (hormones and infection screening). Individual medical assessment is always done. BMI should be under 35 (for the person who will carry the pregnancy), but normal weight is desirable. A partner should also be healthy; a doctor's consultation with medical history and a possible medical examination is included.
Psychosocial Assessment
Treatment may proceed if the future child is deemed likely to grow up under good conditions. According to Swedish law, the assessment includes a conversation with a behavioral scientist, where an in-depth psychosocial evaluation is made and there's an opportunity to discuss the emotional aspect of undergoing sperm donation where the child does not have a genetic connection to both parents.
Matching with a Donor
According to Swedish law, a patient is not allowed to choose the donor's characteristics; the doctor does this. The donor's eye color, hair color, skin color, and body type are matched with the woman who will carry the pregnancy. If it's a couple undergoing treatment together, the donor is matched with the future parent who will not be carrying the child during pregnancy. This way, the child has the potential to resemble both parents.
Insemination
Natural cycle is the most common method for insemination and works well for women with regular menstruation. Ovulation typically occurs about 14 days before the next period. Tablets or FSH stimulation to aid ovulation are used if ovulation is not satisfactory. FSH stimulation is generally only done if the patient does not have her own ovulations and tablet stimulation doesn't work. For insemination, donated sperm that has been frozen for at least six months is used to ensure the sperm is free from viruses. Sperm is thawed and the treatment is done through insemination in connection with ovulation in a normal menstrual cycle or in a hormone-treated cycle. The treatment is individualized according to the woman's own conditions. On the treatment day, the partner or the woman who will carry the pregnancy is inseminated. Usually, the insemination is painless and quickly done. The doctor inserts a thin catheter into the uterus and then the sperm is injected into the uterus. Afterwards, you can live as normal.
When using donated sperm and you have a partner, make sure he/she has given you written consent if he/she cannot be present at the insemination. This is important. By signing the consent, your partner assumes parenthood.
Pregnancy Test
Just over two weeks after the insemination, it's time to take a pregnancy test. Even if a small bleeding occurs, the pregnancy test can be positive. If the pregnancy test is positive, you will be scheduled for an ultrasound examination a few weeks later to confirm that the pregnancy is normal. If the pregnancy test is negative, we plan for continued treatment if you wish.

If you are not inseminated in a natural cycle but are stimulated with tablets or FSH, your abdomen may feel tense and tender due to enlarged ovaries. It is also common to feel emotionally affected during these days.

All donors are healthy, have no hereditary diseases in their family/relatives, have good sperm count, and are between 23 and 45 years old. A prospective donor is tested for several infectious diseases (including HIV 1 and 2, hepatitis B and C, HTLV I/II, and syphilis), and genetic screening is done to ensure the donor is not a carrier of any rare hereditary diseases.
A donor can give rise to children in a maximum of six families in Sweden, and possibly siblings.
The donor is anonymous to the couple, and the receiving couple or woman is anonymous to the donor. However, children born through donation have the right, when sufficiently mature, to find out the donor's identity. The donor has neither obligations nor rights towards the child. Information about the donor is kept for at least 70 years.
