Fertility preservation procedure
Referrals should be faxed to Livio Umeå (fax 010-151 45 26), Kunskapens hus, Lasarettsbacken 7. If the patient is referred from a region other than Västerbotten, a national agreement referral/payment guarantee must be attached, and “Fertility preservation procedure” should be specified in the box for Diagnosis/Question.
It is important that gametes are frozen before treatment begins. Even short-term treatment with cytotoxic agents or radiation can significantly affect fertility.

Note! During the clinic’s Christmas closure (usually from December 24 until Epiphany) and summer closure (usually July), referring parties should contact the switchboard at NUS (Norrlands University Hospital) at 090-785 00 00 for further information about contacting Livio Umeå.
IMPORTANT! Before freezing, infection screening must always be performed for the following analyses, and the test results must be attached to the referral to Livio Umeå. If the sample is to be analyzed at the NUS Microbiology Department, use the referral form for Clinical Microbiology (pink-bordered form). Screening, bloodborne infection IVF, 2504. If embryo freezing is decided, the man must also provide infection screening samples.
